No Filler: How Collagen Beauty Products Can Transform Skin

We hear the word collagen bandied around a lot in beauty, but many of us are probably a bit fuzzy on what exactly it is. We’re not talking about facial fillers, nor are we talking about beauty supplements. We’re focusing on collagen beauty products and how the ingredient can plump skin without a needle in sight. (Those looking to actually stimulate collagen should scan product ingredient labels for collagen peptides or collagen amino acids, which are not the same as collagen found in skin care products.) Instead, collagen contained in beauty products acts as a humectant, a molecule that brings water into skin, helping to plump and hydrate, says Grossman. The ingredient can be used at any age to help moisturize skin.

According to Begoun, there are three sources of collagen: synthetic, animal-derived and plant-derived. Unfortunately, the ingredient label doesn’t indicate which type is in the product, so we’re left to trust the brand’s claims. The good news is, according to research, one type isn’t better than the other and they all help to hold moisture within skin’s surface layers.

The Year of the Rear



In my practice, 2015 could be called “The Year of the Rear” because buttock procedures have exploded in popularity. Both men and women are having procedures done in higher numbers than ever before.

The most commonly requested procedure is known as the Brazilian butt lift. There are a handful of high profile celebrities that deny having it done, while clear photographic evidence shows that they have. Luckily for them and the rest of us, the butt lift operation has undergone a transformation in recent years resulting in a much improved, more reliable end result (pun intended). A desired booty is now within reach for everyone – literally.

A little history of the buttock lift is helpful. Buttock enhancements have been done in a number of ways for many years. Many of these methods included an implant of some sort. It is important to understand that buttock implants can be troublesome, especially because, by necessity, they come in pairs. It always comes down to whether the pair is even. Symmetry is a beautiful thing, and never more so than with the buttocks. And that’s where the Brazilian technique comes in.

Brazilian butt lifts were a popular procedure in 2015. New technology provides more options and better outcomes.The reason symmetry issues rarely occur with the Brazilian butt lift is the fact that the buttocks are sculpted with no implants. As with all plastic surgery procedures, this is where the artistic intuition of the surgeon comes into play. Sculpting is the ultimate expression of art. This is especially true with buttocks. Many patients I see in consultation demonstrate the butt they want by holding their hands in a certain shape. My technique of buttocks shaping moves the fat around, much like what my patients do with their own hands. I take fat from some areas and move it elsewhere in the buttocks. This is sculpting at its best.

The major change or innovation in this area has to do with the way the fat is extracted. The fat taken out is called a graft. It is living tissue that is removed from the body (like a kidney transplant) and replaced (transplanted) into another spot. The problem we used to encounter was due to the heartiness of fat cells. Fat is very fragile and the easily broken cells did not survive the removal and refinement process very well. Some experts say that nearly 50% of the fat we used to take out died before we got it back in. This made buttocks sculpting a little ‘hit or miss.’

Medical advances have now created a new way to remove and refine the fat that ensures that almost all the fat survives the short trip back into the body. First, the fat is removed through teflon coated instruments. This way, the fragile cells slide smoothly through a polished pathway. Second, the fat is concentrated with a low speed centrifuge, which spins the fat and increases the force of gravity to have the heaviest clumps settle into a concentrated area. Third, the fluids, oils and fat cells that didn’t make it are skimmed away, leaving only concentrated, dense, healthy fat cells for insertion. Finally, the fat is injected through a teflon coated instrument, just like the one with which it was extracted. All this is done without exposing the fat to the air, which would ruin many of the cells in the process. The good news is there is also less bruising and swelling with the newer technique.

What about implants? I do them regularly and there is a place for them in creating butt lifts. When a very thin person wants more butt, implants are the only option (other than working out, of course). Also, if a person has literally no butt and desires substantial buttocks, a radical change cannot be done with fat alone. But with implants the incision is bigger, risks are higher and we lose that fine tuning ability of the Brazilian procedure. Nine out of ten patients would do better by moving the fat around.

So now you can better understand how newer techniques for modern buttocks lifting can produce bigger, better, rounder and more reliable results. In the end, you get a fantastic looking backside that both men and women love.

5 Ways Grains Interfere with Weight Loss



There are plenty of reasons why grains can impact your weight loss goals, aside from gluten. We are aware of the obvious benefits of swapping burger buns for whole grains or gluten-free options, but often grains, and wheat in particular, are overlooked as a hidden cause of resistant weight loss.

Let’s take a look at why grain consumption can be related to the dreaded weight loss plateau.

1) It’s a blood sugar bomb. The biggest influence on the body’s ability to burn fat is blood sugar levels. Contrary to common belief, there is minimal difference between whole wheat and white flour. When grains, even gluten-free, are ground into flour from whole kernels, they become dense, quick digesting carbohydrates, causing a rapid rise in blood sugar. A rise in blood sugar tells the body to release high amounts of insulin, a ‘fat storing’ hormone which shuttles sugar into cells and the liver – and the rest into your fat cells. The constant presence of high insulin causes our body to switch from fat burning to fat storing mode. The key is to keep blood sugar from rising too much, too often by consuming lower carbohydrate meals.

2) Wheat contains ‘super starch’. Today’s cross-hybridized wheat contains high levels of the carbohydrate Amylopectin A. Due its fast digestibility, Amylopectin A can raise blood sugar higher than a candy bar. In studies, this super starch has been shown to specifically increase belly fat and lead to insulin resistance.1

3) Inflammatory lectins. Inflammation is a well-known hurdle of fat loss, and wheat has been well documented to have multiple inflammatory properties, one of which is the presence of lectins. Lectins act as a plant’s defense against its natural enemies. The specific lectins found in wheat, wheat germ agglutinin (WGA), stimulate the synthesis of pro-inflammatory chemical messengers called cytokines.2 Due to decades of wheat hybridization, there is increased concentration of these WGA lectins with the highest concentration found in whole grains.

4) Endocrine disrupters. Endocrine disruptors (ED) are chemicals that can interfere with the hormonal system. Xenoestrogens are a type of ED that have estrogen-like effects such as weight gain and have even been shown to increase breast tissue growth in men. Glyphosate, a well-known xenoestrogen, is one of the most highly sprayed toxic additions in our food supply, particularly in wheat crops. An estimated 85% of non-organic wheat crops are sprayed with glyphosate during harvest.

5) Addictive appetite stimulant. Gluteomorphins, named after “gluten” and “morphine,” are opioid-like peptides formed during digestion of wheat. When we consume wheat and your immune system is compromised, these polypeptides can be absorbed into the bloodstream and cross the blood brain barrier, creating a drug-like effect in our brain that not only affects neurologic concerns, but can also cause addictive eating behavior, such as intense cravings and bingeing. This is why going low carb is difficult, and why we have a hard time giving up our favorite bread.

As you can see, there is much more to blame than gluten in grains that could be playing a role in preventing you from losing those last ten pounds.

Guidelines to Your “Perfect Diet”




We would all love to find a convenient way to follow the “perfect diet,” after we figure out what that actually looks like. Many studies agreed on the same healthy diet philosophy. The most popular was published in 2014 by Yale University and identified ‘the best human diet for health’ was to simply eat whole foods.1 This traditional nutrient-dense approach to diet has proven to be effective for those who follow it. However, due to information overload in our society and lack of clear guidelines, implementing a whole food approach on a day-to-day basis can feel overwhelming and unattainable for many.

Here are some easy guidelines to simplify the process of mastering your optimal diet, despite your lifestyle:

Lower carb meals. Aside from some athletes, many health professionals suggest approximately 80% of your meals should lean towards having a lower, or ‘slower’ carbohydrate content.2,3 Slow means the carbohydrates have high fiber content which slow blood sugar spikes. A typical low carb lunch may consist of a large salad, chopped vegetables and beans or chicken. Tip: Don’t skip the healthy fats like olive oil-based dressing or added nuts and seeds.

1/2 plate full of vegetables. Your carbohydrates should come from mostly vegetables. This means replacing your bun and fries with lettuce wrap and a double side of sautéed broccoli. Easy!

Protein. The absolute best appetite suppressant known to date is protein. It also repairs tissues and sustains energy like nothing else. Prioritize and budget for high-quality of protein, especially if from an animal source. Plant-based proteins like beans, lentils, and seeds count too!

Nothing from a bag or a box. Probably not new advice to you, but track how many snacks you consume that are pre-packaged…replace those with veggies and dip, boiled eggs, homemade low-sugar trail mix…real food.

Limit alcohol to 2 days per week. Especially for fat loss; 1-2 drinks max. Nothing thwarts fat loss like alcohol. As a sugary additive to our diet at an average 7 calories per gram, it can easily put your body into fat-storing mode.

The secret behind the above guidelines is not so much that it is offering you the “perfect diet” (still worth following since perfect doesn’t exist!), but it is actually setting your body up for better digestion, increased metabolism for fat burning, and enhanced detoxification. When we provide the body with the optimal nutrition from real food, along with healthy lifestyle habits like exercise and well-managed stress, we can make it easy to manage detoxification, blood sugar and even an occasional ‘off the diet’ splurge.

What Is A Nonsurgical Face-Lift? Plastic Surgeons Explain.



In our current age of social media, in which we’re constantly bombarded with seemingly “flawless” (i.e. filtered) images of Instagram models and the Kardashian-Jenner crew, it’s no surprise that people in this country are more interested in cosmetic procedures than ever before. And there’s a boom in procedures that don’t require patients to go under the knife.

In 2016, the American Society of Plastic Surgeons reported Americans  underwent 17.1 million cosmetic procedures, which was up 3 percent from 2015. Of that 17.1 million, 1.7 million were cosmetic surgical procedures (up 4 percent from 2015) and 15.4 million were minimally invasive procedures (up 3 percent from 2015).

Minimally invasive (and nonsurgical) refers to procedures like facial injectables and laser resurfacing treatments that don’t require hospitalization or large incisions. They’re also associated with little to no downtime, which means you can typically be in and out of the doctor’s office without much discomfort.

Looking at the numbers, it’s clear that while individuals are getting work done, they aren’t always looking to go under the knife. Instead, patients are opting for treatments and procedures that provide results similar to face-lifts without the actual surgery, aka nonsurgical face-lifts.

So, What Exactly Is A Nonsurgical Face-Lift?
A nonsurgical face-lift can include a number of different treatments and procedures that aim to address the effects of aging, as New York-based facial plastic surgeon Michelle Yagoda, explained to HuffPost. One of the most common things people do, she said, is have fillers injected into the face.

As we get older, we lose volume in our faces, specifically in the cheeks, temples, lips, around the mouth and under the eyes, Park Avenue Facial Plastic Surgeon Andrew Jacono explained. With the use of fillers, plastic surgeons are replacing that lost volume to help “reverse the creation of shadows and restore the reflection of light which gives the appearance of youth, energy and vibrancy.”

It should be noted that there are different kinds of fillers one can use ― gel (typically hyaluronic acid, as in Juvederm or Restylane) or fat.

Gel fillers are considered nonsurgical but fat injections are not. With fat injections, doctors take fat from a part of your body, such as your stomach, and inject it into your face to fill in wrinkles and lines. Fat injections last longer than hyaluronic acid fillers and can be permanent, but they do require surgery.

Nonsurgical fillers, on the other hand, can be used in a number of places, including the nasal labial folds, which are the lines that connect your nose to the corners of your mouth; the marionette lines, which extend from the corners of the mouth down to the chin; and in the chin crease, to achieve a more youthful appearance, Yagoda explained. Fillers can also be injected along the jawline to help provide definition, and in the cheek area. Yagoda noted that cheek injections aren’t the right course of action for everyone.

“I think cheek injections are very overused and lead to a lot of premature aging, but in the right person, it can be a really good nonsurgical face-lift,” she said. “That right person is somebody who has a flat mid-face and undefined cheekbones to begin with.”

Yagoda also explained that nonsurgical face-lifts can sometimes involve Botox, which is often used in the forehead.

“Technically it’s not a face-lift but more of a brow lift, but most people think of rejuvenation of the face including all areas,” she said.

“The best thing since sliced bread is Botox,” New York City plastic surgeon Dr. Stafford Broumand of 740 Park Plastic Surgery said, adding that the use of Botox and fillers can shave years off someone’s face, making them look up to “a generation younger.”

What About Procedures With No Needles?
For those who aren’t interested in having needles injected into their faces, there are a number of treatments available that promote tightening of the skin, which can also provide younger-looking skin.

For instance, there are laser resurfacing treatments, such as Clear + Brilliant, that can “change the glow of the skin,” Broumand said. These treatments can tighten the pores and improve the quality of the skin. They come with little downtime, although patients may experience some redness, which generally calms down within a couple of hours, Broumand said.

A chemical peel is another treatment that can help with resurfacing the skin and reducing the look of fine lines, wrinkles and pores, Yagoda said, adding that a chemical peel could provide “up to a 30 percent tightening” of the skin.

There are also treatments that use ultrasound energy or radiofrequency energy to tighten the skin below the surface, such as Ultherapy, ThermiTight and Thermage.

These treatments “address the deep skin layers and foundational layer that lends support to the skin,” Jacono explained. “The foundational layer is called the SMAS (superficial muscular aponeurotic system) and in the neck is called the platysma. With today’s tightening devices, we can bypass the upper layers of the skin and deliver the right amount of energy to the right depths to contract and ultimately lift the SMAS and platysma.”

Yagoda noted that skin-tightening treatments like these can generally be done in two ways.

“There’s the one-and-done treatment, which costs a lot of money. There’s a lot of discomfort during the treatment, and you’re more likely to have a noticeable result,” she said. “And then there’s the multiple treatments that are much more comfortable, kind of like adding a warming treatment on at the end of a facial, and you do a series of those.”

She noted that the multiple-treatment option is less expensive per treatment, “but they add up the more you do,” and you generally see a smaller degree of change. 

How Long Will A Nonsurgical Face-Lift Last?
How long results last will depend on what type of treatment or procedure you get. With nonsurgical procedures, it’s likely you’ll need to get them done regularly to maintain your desired results.

For fillers, Broumand said, it depends on the location of the injection.

“If there’s a lot of motion or movement, the fillers tend to dissipate more rapidly. If there’s less motion in the area it’s injected, they tend to stay longer,” he said, adding that there are different types of fillers with different formulations, which will also affect how long it lasts. The effects of Botox generally wear off in 12 weeks, Broumand said, noting that patients will generally come in regularly to maintain their results.

Your practitioner should advise you on when to return for a follow-up visit, but, as Broumand suggested, most patients will be able to tell on their own, as they’ll notice the treatments wearing off.

As both Broumand and Yagoda explained, there can be cumulative benefits to receiving injectables regularly.

“The more diligent our patients are about maintaining it regularly, the less they need over time, because those muscles are affected and the wrinkles are diminished, so over time, what [initially] required three syringes [might] now require two syringes,” Broumand said.

“I find typically with my patients, the first and second time they need the most volume of filler and then, after that, the touchups are a lot less,” Yagoda said. “The body makes a type of scaffolding where the filler is injected and that allows the wrinkle to not get as deep as it was initially.”

With regular maintenance and proper treatment, Broumand said that patients who start in their 30s can look the same in their 50s.

When it comes to the laser treatments, Yagoda said, when they work, they can be fairly effective. However, she said, “time doesn’t stand still. The aging process continues.”

“What you’ve tightened will stay tight up until a certain point, but your skin and soft tissue are going to continue to age,” she added. “So you will kind of need to do more at a certain point to catch up with the aging process.”

Jacono explained that nonsurgical face-lifts are “a temporary fix” that can last one to two years, whereas a surgical face-lift typically lasts 10 years.

“As we age and our facial skin becomes more lax, an increasing amount of injectable material and tightening is needed for the increasing void of volume, but the results will show a decreasing amount of improvement,” he said. “It is at this point that patients usually opt for surgery.”
